Medical Scribe is an assistant to the doctor who is responsible for recording patient's medical histories or any information dictated to them. A medical scribe is responsible for gathering information and documentation for patients during their visit to help Doctors and other medical staff process patient as efficiently as possible.

                                         

    One of the most important skills that a Medical Scribe needs is their ability to listen. They must also be able to quickly document what physicians say during a visit and then accurately type on their workstation to record notes and procedure notes on behalf of medical staff.

                                             Medical Scribes work directly with physician and Doctors to transcribe information during clinical visits in real time into electronic health records (EHRs). They also work directly with patients to document and record information from their charts during their clinical visit.

A job responsibilities of Medical Scribe includes:

• Prepares and assembles medical record documentation 
• Enters the patient room with the physician during patient visit to capture and transcribe medical record documentation utilizing electronic medical record application
• Ensures medical record compliance by self documentation attestation.
• Performs other clerical duties and tasks to improve provider productivity and clinical workflow as assigned.  
• Updates patient history, physical exam and other pertinent health information in the patient. 

                                           Depending on which area of practice the scribe works in the position may also be called clinical scribe, ER scribe, ED scribe or just scribe. A scribe can work on site or remotely. Medical Scribes who work at an off-site location are known as virtual medical scribes.

                                          ER scribe works in the emergency department of a hospital. Their duties may include overseeing the documentation of each patient's visit to the ED and acting as the physician's personal assistant . A scribe might work with one physician 's per shift or might be shared between multiple providers, depending on the agency.
